This list is meant to be a brief summary of the benefits available to PHCS employees.

 

PHCS offers its employees the opportunity to participate in a variety of important and a comprehensive benefit plans, including group medical and dental insurance, life insurance, disability insurance, paid time off, flexible spending accounts, tuition reimbursement, 401(k) among othersand much more. A brief summary of each benefit is included below.

Below is a brief summary of the benefits available to PHCS employees.

 

 

¨      Paid Time Off (Vacation, Sick Time, Personal Time Off) Employees accrue 165 hours per year; equivalent to 22 days based on our standard 37.5 hour per week pay period. After 5 years of service, employees accrue 202.5 hours per year; equivalent to 27 days. Maximum PTO accrual is 225 hours.  Generous Cash Out provisions are available.

-         PTO accrual is as follows (based on our standard 37.5 hour work week):

¨      Holidays
Nine per year - New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, day after Thanksgiving, Christmas Day and two holidays selected prior to the beginning of each year.

¨      Health Insurance
Choice of Preferred Provider Option or Point of Service Option for a family or individual, which includes prescription coverage.  Premiums are paid through payroll deduction on a pre-tax basis.

¨      Dental Insurance
Individual or family coverage for preventative care, minor and major restoration and orthodontia. Premiums are paid through payroll deduction on a pre-tax basis.

¨      Vision Insurance
Basic vision is provided with the additional choice of two low cost, employee paid supplemental vision care plans through Youdecide.com

¨      401(k) Retirement Plan
Pre-tax payroll deduction to your own a retirement account.  Choice of fifteen investment options administered by Fidelity Investments. Company match of 100% of first 3% of eligible contributions up to regulatory maximum contribution.

¨      Flexible Spending Account (FSA): – Dependent Care
Pre-tax payroll deduction to reimburse employees for eligible dependent care expenses.

¨      Flexible Spending Account (FSA) – Medical and Dental Expenses
Pre-tax payroll deduction to reimburse employees for eligible out-of-pocket medical and dental expenses.

¨      Life Insurance
Company paid life insurance equal to two times annual base salary.

¨      Optional Life Insurance
Additional employee paid life insurance options for the employee, spouse and/or dependent children.

¨      Tuition Reimbursement
Reimbursement for college courses, CEU programs required to maintain a professional license and certain certificate programs. Maximum annual reimbursement is $6,000 per year for graduate school, $3,500 for undergraduate programs and $2,000 for eligible CEU courses. Some tuition reimbursements may be subject to taxes in accordance with IRS regulations.

¨      Disability Insurance – Short Term (STD)
65% salary replacement for qualified short-term illness/accident for up to 13 weeks following a two week waiting period. 100% company paid.

¨      Disability Insurance – Long Term (LTD)
60% salary replacement program for qualified long-term illness/accident after STD is exhausted. 100% company paid.

¨      Employee Assistance Program (EAP)
24-hour hotline for assistance in dealing with issues related to stress, finances, work, family, etc. Coverage includes up to three meetings with a counselor as required to providing counseling and assistance to the employee and to family member(s). 100% company paid.

¨      Accidental Death and Dismemberment Insurance
Two times annual salary, rounded to the nearest thousand. 100% company paid.

¨      Business Travel Accident Insurance                 
Additional life insurance covers up to $250,000. 100% company paid.

¨      Credit Union
Automatic payroll deduction to the Metropolitan Credit Union. Membership includes many preferred programs, such group auto insurance and discounts on new and used cars.

¨      OnSite -site fitness center (Waltham MA)
Full-service exercise facilities. Membership discounts given to PHCS employees.
